1. Based on observation and interview it was determined the facility failed to properly maintain one oxygen storage area on one of two levels in this component.

Findings include:

a. Observation of the first floor medical gas manifold room at 9:53 AM, on February 2, 2010, revealed four "H" sized cylinders of nitrous oxide and two "E" sized cylinders of oxygen were not properly secured.

Interview with maintenance representative #1 at 9:54 AM, on February 2, 2010, confirmed this condition.

1. Based on observation and interview it was determined the facility failed to limit the use of unauthorized electrical components in one instance in this component.

Findings include:

a. Observation of the emergency department office at 9:46 AM, on February 2, 2010, revealed an unauthorized extension cord was used to power the coffee pot.

Interview with maintenance representative #1 at 9:47 AM, on February 2, 2010, confirmed this condition.
