Based on observation and staff interview, the facility failed to ensure 2 corridor doors were resistant to the passage of smoke in 1 of 3 smoke compartments. The findings were:

Observation on 3/15/11 at 12:55 PM revealed rooms #809 and #810 (doors into a patient examination room and a physician's office) had a self-closure device attached, but it had been disabled. Interview with the plant operations manager at the time of the observation confirmed the observation.

Observation on 3/15/11 at 12:08 PM revealed one "E" sized oxygen tank standing in an upright position in the doorway to patient room #108. The tank was without restraint or other device to prevent it from falling over. Interview with plant operations manager at the time of observation confirmed the tank should be secured.
