Based on document review and interview, the facility failed to include in the reassessment of its discharge planning process, an evaluation for preventable readmissions to ensure the facility is responsive to the discharge needs of all patients.

Findings included:

1. Review of the facility's evaluation of its discharge planning process for 2012 lacked evidence that preventable causes of readmissions are being evaluated during 2012.
2. Interview with B#2 on 6-26-12 at 1430 hours indicated that evaluating readmissions for preventable causes has not been done at the hospital during 2012.
3. Interview with B#5 on 6-27-12 at 0920 hours confirmed the hospital has not been evaluating readmissions for preventable causes during 2012, but plans are in place to reinstate the practice later 2012.
