On the day of the validation survey, based on direct visual observations of the facility and interview with staff from 10:15 a.m. until 2:30 p.m., the facility failed to ensure that the construction for the hazardous areas were being maintained in accordance with code.
Findings Include:
1. The mechanical room door next to the second floor stairwell had tape over the strike plate which prevented it from latching.
2. The two educational storage rooms that were greater than 50 sq. ft. located on the first floor were not self closing when tested.

On the day of the validation survey, based on direct visual observations of the facility and interview with staff from 10:15 a.m. until 2:30 p.m., the facility failed to ensure that the construction for the laboratory area was being maintained in accordance with code.
Findings Include:
The 1 hour fire rated wall that seperated the lab from the corridor had open unprotected penetrations above the ceiling also the exit door that exited into the corridor was not a 3/4 hour fire rated door.
