Based on observation, it was determined that the facility failed ensure that continuous exit lighting was provided.

Findings include:

1. On 3/13/12 at 11:00 AM, in the presence of Staff #2, the exit sign was not lit in the #2 exit of the basement stair.

Based on observation, it was determined that the facility failed to ensure that the automatic sprinkler system was continuously maintained.

Findings include:

1. On 3/14/12 at 2:00 PM, in the presence of Staff #2, eight (8) boxes of supplies were stored within 18" of the sprinkler system in the EPS storage room.

Based on observation, it was determined that the facility failed to ensure that medical gas storage areas are protected.

Findings include:

1. On 3/13/12 at 10:25 AM, in the presence of Staff #2, four (4) tanks of oxygen were unsecured in the basement cylinder storage room.

2. On 3/13/12 at 10:40 AM, in the presence of Staff #2, nine (9) tanks of gas were unsecured in the tank farm.

Based on observation, it was determined that the facility failed to ensure that flammable liquids are stored in approved containers.

Findings include:

1. On 3/13/12 at 10:10 AM, in the presence of Staff #2, eight (8) cans of flammable paint were stored on a shelf in the non-flammable paint storage area.
