Based on observation, staff interview and document review, it was determined that the facility failed to implement infection control measures to mitigate the risk of COVID-19 transmission.
Findings include:
Reference #1: Facility policy titled "Temperature and Symptom Screening of Visitors" states, " ...Procedure ...Upon entrance, all visitors will undergo a temperature and symptom screening ..."
Reference #2: Facility document titled "COVID-19 2021" states, " ...Physical Distancing/Barriers/Ventilation ...
1. On 10/20/2021 at approximately 9:30 AM, two (2) Department of Health (DOH) staff entered the facility. Upon entering the facility, the DOH staff were not screened for COVID-19 symptoms.
2. At 10:42 AM, Staff #9 stated that visitors that are attending a doctor's appointment are not screened upon entrance instead they are screened in the doctor's office.
3. At 11:20 AM, Staff #11 was observed wearing a cloth face mask. This writer interviewed Staff #11. During the interview, Staff #11's mask was noted to be loosely fitted. Staff #11's entire nose was visible at times during the interview.
4. In the Courtyard area at 12:30 PM, four (4) staff were observed sitting at a table, in a high traffic area, unmasked and engaging in conversation.
5. The above findings were confirmed with Staff #6.
