Based on observation and interview the facility failed to maintain the physical environment for one of one therapy pools observed resulting in the potential for patient harm. Findings include:

During the inspection of the outpatient facility on October/9/2017 at approximately 1030 about 10 square tiles sized 1.5" x 1.5" were missing from the last step leading to the floor of the swimming pool floor. The tiles could pose a potential hazard and may cause a cut and or injury to all using the pool. Missing tiles finding was confirmed by accompanying staff A and B on 10/9/2017 at approximately 1030.
