Findings:
The inspector observed, while accompanied by the CEO, during the hours of the inspection from approximately 11:30 a.m. to 2:30 p.m. on 08/01/2023, that there were the following issues:
1) The door to the Electrical Room (near the Staff Lounge and Pantry), which is located in the means of egress, had parts of the self-closer mechanism removed, thus rendering it not self-closing.
2) The door to the Storage Room (near the Janitor's Closet), which is located in the means of egress, did not close when testing the self-closing mechanism.

The inspector observed, while accompanied by the CEO, during the hours of the inspection from approximately 11:30 a.m. to 2:30 p.m. on 08/01/2023, that there were the following issues:
1) The 1-hour fire rated Soiled Utility/Linen Room, which is located near the Public Toilet and the Dining Room, had walls that did not go up to the bottom of the deck. It couldn't be verified if current ceiling assembly meets requirements of 1-hour fire rating resistivity.

1) The interstitial space created between the ceiling and the bottom of the roof deck contained combustible materials (wood) that was used to support Heating, Ventilating and Air Conditioning (HVAC) equipment, and was not provided with fire sprinkler protection.

Findings:
The inspector observed, while accompanied by the CEO, during the hours of the inspection from approximately 11:30 a.m. to 2:30 p.m. on 08/01/2023, that there were the following issues:
1) Barrier walls had unsealed penetrations including conduit sleeves, cables, and conduits, and unsealed openings. Ensure all penetrations and openings in rated construction are sealed and repaired (patched) using an industry standard fire stop system.
