Based on observation, record review, and interview, the facility failed to involve all contracted services in their quality assessment and performance improvement program in 2 of 2 contracted services reviewed (Radiology and Laundry Services) and failed to implement an ongoing program of quality improvement that included all departments of the facility 4 of 16 (Laundry, Radiology, Environmental Services and Organ Procurement).
