Based on interview and record review, the facility failed to obtain a consent to treat for 3 (#s 3, 7 and 20) of 20 sampled patients. Findings include:
1. Patient #3 was admitted on 3/8/18 and expired on 6/6/18.
Review of patient #3's medical record failed to show a consent to treat form had been signed by the patient or the patient's representative.
2. Patient#7 was admitted to the emergency room on 3/29/19.
Review of patient #7's medical record failed to show a consent for treatment had been signed by the patient or the patient's representative.
3. Patient #20 was admitted on 11/26/18 and discharged on 11/30/18.
Review of patient #20's medical record failed to show a consent to treat form had been signed by the patient or the patient's representative.
During an interview on 5/15/19 at 4:00 p.m., staff members C and G stated part of the process for admission to the ER or inpatient was to obtain a consent to treat form. Staff members C and G stated if the patient presented in an emergent situation, the consent would be obtained when the patient was able to sign. Staff members C and G stated if a patient was incapacitated, then every effort would be made to obtain a consent to treat from the patient's representative.
