Based on review of medical records, policy and procedures and staff interview it was determined the facility failed to ensure a Registered Nurse supervised and evaluated the nursing care related to positioning for 1 (#3) of 3 patients sampled. This practice does not ensure the patients safety, goals, and needs are being met.
Findings include:
Patient #3's nursing admission assessment dated 5/26/10 revealed the patient was unable to turn independently in bed and required assistance for turning and repositioning. Review of the patient's plan of care initiated on 5/26/10 revealed turning and repositioning to prevent skin breakdown. Review of the nursing documentation revealed inconsistent turning and repositioning. Review of the nursing documentation revealed on 6/29/10-7/09/10, 7/11/10-7/20/10, 7/22/10-8/14/10, and 8/26/10-8/27/10 there was no documentation of the patient being turned and repositioned.
Review of the facility's policy, "Pressure Wound Treatment" stated in order to eliminate or minimize the wound the patient will be turned every two hours according to established facility routine.
Interview with the DON (Director of Nursing) on 9/03/10 at 11:15 a.m. confirmed there was no documentation of the turning and repositioning.

Based on observations and interviews it was determined the facility failed to ensure the overall hospital environment was maintained in such a manner that the safety and well-being of patients are assured.
Findings include:
1. A tour of the medical/surgical wing on 9/2/10 at approximately 2:00 p.m. was conducted. During the tour observations were made of the bathroom shared by rooms 212 and 214. A rusty shower drain that protruded approximately 1 inch above the drain inset was noted.
2. Observations made in room 213 revealed stained ceiling tiles above the shower, a dirty shower pan with bio growth in the corners of the shower and stained ceiling tiles in patient room 213.
An interview conducted on 9/02/10 at 3:00 p.m. with the Director of Nursing that confirmed the findings.
