Based on review of facility documentation, staff interview and confirmation, the facility failed to meet the requirements for life safety from fire specifically the applicable provisions on the 2000 edition of the Life Safety Code of the National Fire Protection Association. Potentially all patients, visitors and staff could be adversely affected. The facility had a census of 86 patients at the time of the post survey revisit .

Findings included

The main building of the facility failed to meet K67, which addressed that fire/smoke dampers were to be tested in accordance with the National Fire Protection Association 90A, 1999 edition, section 3-4.
