Based on record review and staff interview, it was determined the hospital failed to ensure patients who are transferred, are provided with a notice of patient's rights. This failed practice impacted three (3) of six (6) patients reviewed (patients # 9, 10, 11), and has the potential to result in a violation of rights for any patient who is transferred into the hospital.
Findings include:
1. Review of the medical record for patient #9 revealed the patient was admitted through transfer on 3/19/13. Review of the record revealed there was no documentation to reflect the patient had been informed of the patient's rights.
This record was reviewed and discussed with the Clinical Manager at 0940 on 3/20/13, and she acknowledged the record did not reflect the patient was informed of patient's rights.
2. Review of the medical record for patient #10 revealed the patient was admitted through transfer on 3/11/13. Review of the record revealed there was no documentation to reflect the patient and/or decision maker had been informed of the patient's rights.
This record was reviewed and discussed with the Clinical Manager at 1000 on 3/20/13, and she acknowledged the record did not reflect the patient and/or decision maker was informed of patient's rights.
3. Review of the medical record for patient #11 revealed the patient was admitted through transfer on 3/16/13. Review of the record revealed there was no documentation to reflect the patient and/or decision maker had been informed of patient's rights.
This record was reviewed and discussed with the Clinical Manager at 1130 on 3/20/13. She agreed the record did not reflect the patient and/or decision maker was informed of patient's rights.
