Based on observations it was determined the Hospital failed to ensure all rooms in which patient care/activities occurred were maintained to provide a safe environment.
Findings included :
During a tour of the nursing unit on 2/25/10, at 10:45 AM it was observed there were exposed radiator pipes in the dinning and activity room, the ADL kitchen, ADL bathroom, the gym and in rooms on the adjacent wing, that had been temporally allocated for treatment of outpatients.
