PATIENT RIGHTS: CARE IN SAFE SETTING

Based on review of personnel files and interview, it was determined the facility failed to ensure patients received care in a safe setting in that 2 of 5 RN's (Registered Nurses) #1, #4 had no proof of current BLS (Basic Life Support) Certification. Failure to assure all RN's maintained current training in life saving measures did not assure staff proficiency in life saving measures and had the potential to affect all patients receiving care in the facility. Findings follow:
A. Review of personnel files on 4/13/22 from 11:15 AM to 12:05 PM, showed 2 of 5 RN's (#1, #4) had no proof of current BLS certifications.
B. During an interview with the CEO (Chief Executive Officer) on 4/13/22 at 12:05 PM, she stated and confirmed that 2 of 5 RN's did not have proof of current BLS Certification.

Based on review of personnel files and interview, it was determined the facility failed to surveille and avoid sources and transmission of infection by not screening potential employees for proof of Hepatitis B immunization or declination of the vaccine for 11 of 12 (RN (Registered Nurse) #1, #2, #3, #4, #5, LPN (Licensed Practical Nurse) #1, #2, #3, CNA (Certified Nursing Assistant) #1, BHA (Behavioral Health Assistant) #1, #2) patient care staff. Failure to screen for Hepatitis B immunization/declination did not assure patients would be protected from all forms of HAI's (Healthcare Acquired Infections) and had the potential to affect all patients receiving care in the facility. Findings follow:
A. Review of personnel files on 4/13/22 from 11:15 AM to 12:05 PM, showed no screening for Hepatitis B proof of immunization or declination for 11 of 12 (RN (Registered Nurse) #1, #2, #3, #4, #5, LPN (Licensed Practical Nurse) #1, #2, #3, CNA (Certified Nursing Assistant) #1, BHA (Behavioral Health Assistant) #1, #2) patient care staff.
B. During an interview with the CEO (Chief Executive Officer) on 4/13/22 at 12:05 PM, she stated and confirmed that there was no proof of Hepatitis B screening for the above patient care staff.
