Based on observation during the survey of the facility on 04/12/23 with the Director of Plant Operations, the facility failed to provide the required fire resistance at rated walls with proper identification at several locations. It appeared this was primarily due to unsealed communication wiring and conduit. This condition does not meet the following requirements:

NFPA 101: 8.3.5.1 ..... Penetrations for cables, cable trays, conduits, pipes, tubes, combustion vents and exhaust vents, wires, and similar items to accommodate electrical, mechanical, plumbing, and communications systems that pass through a wall, floor, or floor/ceiling assembly constructed as a fire barrier shall be protected by a firestop system or device.

NFPA 101: 8.5.2.2 ..... Smoke barriers shall be continuous through all concealed spaces, such as those found above a ceiling, including interstitial spaces.

NFPA 101, 8.7.1.3 .....Doors in barriers required to have a fire resistance rating shall have a ¾ hour fire protection rating and shall be self-closing or automatic closing in accordance with 7.2.1.8 This space must be ventilated as though it were storage space to comply with 25 TAC, HLR, 2007: §133.169 [C].

NFPA 1013: 18.3.2.1 ..... Any hazardous areas shall be protected in accordance with Section 8.7, and the areas described in Table 18.3.2.1 shall be protected as indicated. The doors shall be a minimum ¾ hour fire rated doors set in rated frames.

HLR 2007: §133.162(d)(2)(B)(v) .....Floor, wall and ceiling penetrations by pipes, ducts, and conduits or any direct opening shall be tightly sealed to minimize entry of dirt particles, rodents and insects. Joints of structural elements shall be similarly sealed.
