Based on staff interview and clinical record review it was determined that the nursing staff failed to administer medications as ordered by the physician for 1 (#1) of 3 sampled records. This practice does not ensure safe and effective medication therapy.
Findings include:
Patient #1's physician order dated 9/27/10 instructed for Zosyn (an antibiotic) 4.5 grams Intravenous (IV) every 8 hours. Review of the Medication Administration Record (MAR) for 9/28/10 at 6:00 a.m. revealed no documentation the Zosyn was administered. Review of the MAR for 10/01/10 at 6:00 am revealed no documentation the Zosyn was administered. Nursing failed to follow physician orders for administration of the antibiotic.
Interview on 1/10/11 at 11:00 a.m. with the Risk Manager confirmed there was no documentation the medication was administered.
