Based on observation and staff interview, the Critical Access Hospital (CAH) failed to install a backflow prevention device on 1 of 1 water supply line with an attached hose in the laundry department (floor sink in restroom). Failure to have a backflow prevention device on a water supply line placed the patients and the public at risk of contamination of the public water supply.
Findings include:
Observation of the restroom in the laundry department at 9:25 a.m. on 01/19/17, revealed a water supply line with an attached hose below the top rim of the floor sink. Observation failed to identify a backflow prevention device on the water supply line.
During interview at 9:30 a.m. on 01/19/17, a maintenance staff member (#1) confirmed the water supply line to the floor sink in the laundry department restroom did not have a backflow prevention device.
