1. On 1/6/14 at 10:30 AM, in the presence of Staff #4, an ABHR was mounted above an electrical outlet in the pre-admission corridor.

2. On 1/9/14 at 10:10 AM, in the presence of Staff #4, an ABHR was mounted above an electrical outlet in Room #303.

3. On 1/9/14 at 10:25 AM, in the presence of Staff #4, an ABHR was mounted above an electrical outlet in Room #304.

4. On 1/9/14 at 10:30 AM, in the presence of Staff #4, an ABHR was mounted above an electrical outlet in Room #310.

5. On 1/9/14 at 11:05 AM, in the presence of Staff #4, an ABHR was mounted above an electrical outlet in Nursery #1.

6. On 1/9/14 at 11:25 AM, in the presence of Staff #4, an ABHR was mounted above an electrical outlet in Room #488.
