Based on observation during the facility walk through with the maintenance supervisor on 06/30/2010, it was noted that all cross-corridor pairs of smoke barrier doors on both floors, lacked astragals across the meeting edges. The opening between the edges of the pairs of doors exceeded 1/4 inch in all instances observed, which is in excess of the 1/8 inch maximium allowed.

(a); Based on observation during the facility walk through with the maintenance supervisor on 06/30/2010, it was noted that the electrical power for the fire alarm system was provided from the normal power supply, instead of the emergency electrical system. NFPA 99(1999) 3-4.2.2.2(b)3a requires that fira alarm systems be powered from the Life Safety branch of the Emergency Electrical System (EES).
(b); Based on observation during the walk through with the maintenance supervisor on 06/30/2010, it was also noted that the face of the fire alarm control panel lacked any labeling identifing the identification of the electrical panel and circuit number supplying power to the fire alarm system as required by NFPA 72 (1999) 1-5.2.5.2.

Based on a review of the records during the onsite survey of 06/30/2010, it was noted that an annual load test of the emergency generator was not documented. Further discussion with the safety officer and maintenance supervisor revealed that generator load testing had not been conducted for a minimum of thirty minutes at least monthly under 30% loading, or annually for two hours under full load conditions as required by NFPA 110(1999)8.4.1.1 & 8.4.2.
