Based on review of policies and procedures, medical records and staff interview, it was determined that the social worker/case manager failed to conduct within 24 hours, an initial discharged planning assessment for four (4) of nine (9) patients records reviewed.

Review of facility's policy entitled "Care Management (SW/NCM)" last revised 4/2012, revealed that care management would document an initial discharge planning assessment within 24 hours of admission to the unit/floor or on the first business day.

Review of the following patients' records revealed as follows:

1) Patient #4 was admitted on 4/9/16 at 1:31 p.m. and the initial discharge planning assessment was performed by the social worker/case manager on 4/13/16 at 6:18 p.m.

2) Patient #6 was admitted on 4/29/16 at 9:13 p.m., and did not have an initial discharge planning assessment until 5/18/16 at 1:23 p.m.

3) Patient #7 was admitted on 4/30//16 at 7:11 p.m. and the initial discharge assessment was conducted on 5/5/16 at 4:10 p.m.

4) Patient #8 was admitted on 5/21/16 at 8:44 p.m. and the initial discharge assessment was not conducted until 5/25/16 at 3:29 p.m.

During an interview on 6/13/16 at 10:00 a.m., in a facilty conference room the Interim Patient Safety Officer acknowledged the above findings.
