Based on observation the facility failed to ensure that all valves of the sprinkler system were electronically supervised and connected to the alarm system for two devices, a shut off to the stand pipe and an exterior post indicator valve which control water flow the main sprinkler system. This affects all patients in a census of 245.
Findings included:
Observation on 03/17/10 revealed two devices, shutoff valves to the sprinkler system, that were without an electronic supervision device to indicate if the water flow had been shut off or the device had been tampered with. One of the devices was an exterior valve (post indicator valve) to the building which controlled the water flow to the main sprinkler and was locked shut. The second device was identified as #41, a valve to the stand pipe system in a stairwell.

Based on observation the facility failed to ensure that all compressed medical gasses were stored appropriately to prevent falling. The tanks were not individually secured.
Findings included:
Observation on 03/16/10 revealed an outside storage area with "H" size tanks inside a locked chain link fence. The majority of the tanks were in individual racks. A group of 10 tanks were secured by a single chain to the chain link fence. These 10 tanks were not individually secured.
Chapter 5.1.3.3.2 of the 2002 edition of the Life Safety Code (NFPA 99) published by the National Fire Protection Association states that locations for the central supply systems and the storage of medical gas shall meet the following requirements: (7) be provided with racks, chains or other fastenings to individually secure all cylinders, whether connected, unconnected, full, or empty from falling.
