Based on observation and interview the facility failed to ensure compressed medical gasses were secured to prevent falling. The hospital census was eight Acute Care patients, six Swing Bed patients and three Observation (Observation - outpatient services provided to a patient while the patient's physician decides whether to admit the patient to Acute Care services or discharge the patient) patients for a total census of 17.
Findings included:
1. Observation on 05/21/24 at 11:00 AM in the medical gas storage room showed two large tanks and three smaller tanks unsecured.
During an interview upons observation Staff H, Manager of Facilities, confirmed the tanks in the med gas room were not secure from falling.
2. Observation at 4:00 PM showed 12 large tanks in an outdoor storage area secured by only one strap which showed fraying along the length of the strap.
3. On 05/22/24 at 8:10 AM Staff H confirmed the tanks in the outdoor storage area were not reliably secured from falling.
