Based on document review, observation, and interview, it was determined that the Hospital failed to adhere to the recommended infection control guidelines to prevent and control the transmission of COVID-19, by not ensuring that social distancing (at least 6 feet apart) was maintained during group therapy.
Findings include:
1. On 9/21/2020, the Hospital's Infection Control Plan (2020) and COVID-19 readiness plan (dated September 20, 2020) were reviewed. The documents lacked a policy to address social distancing guidelines while in common areas and during group activities.
2. CDC's (Centers for Disease Control and Prevention) "Clinical Care Guidance for Healthcare Professionals about Coronavirus (COVID-19)", (updated July 16, 2020) was reviewed on 9/22/2020, and included, "Encourage Physical Distancing: Healthcare delivery requires close physical contact between patients and HCP. However, when possible, physical distancing (maintaining 6 feet between people) is an important strategy to prevent SARS-CoV-2 transmission. Examples of how physical distancing can be implemented for patients include: Modifying in-person group healthcare activities (e.g., group therapy, recreational activities)...scheduling smaller in-person group sessions while having patients sit at least 6 feet apart."
3. On 9/22/2020 at 9:15 AM, a tour of the Outpatient Behavioral Health Unit was conducted. During the tour, group therapy sessions were being conducted. One of the group rooms had 4 patients and a Mental Health Technician. Three of the four patients were seated in close proximity to each other, and social distancing guidelines were not maintained.
4. On 9/22/2020 at 9:30 AM, an interview was conducted with the Outpatient Director (E # 6). E # 6 stated that social distancing should be practiced and maintained during groups. E # 6 stated that they try to adhere to social distancing guidelines by having the patients sit in every other seat. E # 6 acknowledged that the 3 patients that were in group were not distanced 6 feet apart, and they should have been.
5. On 9/22/2020 at 9:52 AM, an interview was conducted with the Infection Control/Employee Health Nurse (E #3). E #3 stated that the Hospital does not have a policy to address social distancing while in common areas and during group activities.
