Based on interview and record review the hospital failed to ensure thorough nursing evaluation and care was provided to 1 of 1 (Patient #1) patients, in that the patient was discharged home with her IV (intravenous) catheter still inserted in her left forearm.
Findings included:
Patient #1's medical record indicated the patient was admitted to the hospital on 5/12/19 for diarrhea, syncope, and falls. She had an IV catheter placed in her peripheral left forearm on 5/15/19 at 1311. There was no documentation of the removal of the IV catheter before her discharge.
During an interview on 8/21/19 at 10:30 AM with Personnel #1 she said she was aware of the IV catheter being left in Patient #1's forearm. Personnel #1 had spoken with the patient's daughter who said that the IV catheter was still inserted after the patient was discharged home. The daughter had to remove the IV catheter from the patient. The surveyor reviewed the patient's medical record with Personnel #1. Personnel #1 confirmed there was no documentation that the left forearm IV catheter was removed from the patient before Patient #1 was discharged from the hospital.
