A review of 10 medical records (6 open and 4 closed) revealed that one patient did not receive adequate medications during his inpatient stay. Patient #6 was admitted after surgical complications. Medication reconciliation of home medications was done on admission and at discharge. Patient #6 was taking Advair at home for a history of asthma. This medication was not ordered during his hospital stay. The patient was instructed at discharge to continue with this medication at home.
