Based on findings from observations, there were penetrations through the fire resistive walls of the electrical closet in the North Wing in the Neonatal Intensive Care Unit (NICU) and above the ceiling in the fire resistive wall of the clean room adjacent to birthing room 1 in the Birthing Center, both on Level 5 of Wilson Medical Center.
Penetrations diminish the fire resistive capabilities of the walls and must be properly sealed.
Findings include:
-- Per observations at 11:06 a.m.on 4/29/10, penetrations were observed through the fire resistive walls of the electrical closet in the North Wing in the Neonatal Intensive Care Unit (NICU) on Level 5 of Wilson Medical Center.
-- Per observations at 11:35 a.m.on 4/29/10, penetrations through the fire resistive wall above the drop ceiling were observed in the exterior clean room wall, located next to birthing room 1 in the Birthing Center.
-- The UHSH Facilities Coordinator was present at the time the observations were made and verified the presence of the penetrations at each location.

Based on findings from observations, there was no self-closing mechanism on the door of the electrical closet in the North Wing of the Neonatal Intensive Care Unit (NICU) on Level 5 of Wilson Medical Center (WMC).
Findings include:
-- Per observations at 11:06 a.m. on 4/29/10, there was no self-closer on the electrical closet door in the NICU on Level 5 of the North Wing of WMC.

Based on findings from observations, two exits remote from each other were lacking in the medical records storage area in the sub-basement of the North Tower at Wilson Medical Center.
Findings include:
- - Per observations at 4:35 p.m.on 4/29/10, the two exits in the medical records storage area of the sub-basement of the North Tower were not located remote from each other. The UHSH Facilities Coordinator was present at the time the observation was made and verified the two exits were not remote from each other. The medical records storage area, which is approximately 3000 sq. ft., may be considered a fire section of this floor of the building.

Based on findings from observations, the following egress corridors were not maintained free of obstructions: the 6th floor corridor in the west wing of the Memorial Building of Binghamton General Hospital (BGH), the 4th floor corridor of the ambulatory surgery suite in the Park Building of BGH, the 3rd floor corridor in the west wing of the Memorial Building of BGH, and the north corridor of Level 4 of Wilson Medical Center.
Findings include:
-- Per observations at 11:45 a.m. on 4/28/10, the 6th floor corridor of the west wing of the Memorial Building at BGH contained 4 soiled linen carts, 3 computerized medical stations, and 1 housekeeping cart. The UHSH Facilities Coordinator was present at the time the observation was made and confirmed the presence of these items in the corridor.
-- Per observations at 1:40 p.m. on 4/28/10, the corridor in the ambulatory surgery suite at BGH contained 1 adult bed, 1 child's bed, 3 computerized medical stations, 1 cuff station, 1 general purpose cart, and 1 storage cart. The UHSH Facilities Coordinator was present at the time the observation was made and confirmed the presence of these items in the corridor.
-- Per observations at 3:15 p.m. on 4/28/10, the 3rd floor corridor in the west wing of the Memorial Building at BGH contained 3 computerized medical stations, 3 soiled linen carts, and 1 clean linen cart. The UHSH Facilities Coordinator was present at the time the observation was made and confirmed the presence of these items in the corridor.
-- Per observations at 12:25 p.m. on 4/29/10, four fold-up beds were observed in the north wing of the 4th Level of Wilson Medical Center. The beds restricted access to room 477.
