Based on observation, documentation review, and staff interview, the hospital failed to ensure illuminated exit signs were installed at all required locations (K293). The hospital failed to ensure hazardous area one hour fire rated construction was maintained (K321). The hospital failed to ensure sprinkler heads were properly maintained (K353). The hospital failed to ensure there were no penetrations in the smoke barriers (K372). The hospital failed to ensure electrical outlets were properly installed in regard to GFCI (K912). The hospital failed to ensure remote annunciator panels were installed (K916).

Based on observation, documentation review, and staff interview, the hospital failed to meet the provisions of the Life Safety Code of the National Fire Protection Association (NFPA) 101, 2012 Edition, Chapter 19 Existing Health Care Occupancies. This had the potential to affect all patients receiving services from this facility. The facility census was 281.
Findings include:
K293 - Illuminated exit signs were not installed all required locations;
K321 - Hazardous area one hour fire rated construction was not maintained;
K353 - Sprinkler heads were not properly maintained;
K372 - Penetrations were found in the smoke barriers;
K912 - Electrical outlets were not properly installed in regard to GFCI; and
K916 - There were no remote annunciator panels installed.
