Findings include:
1. On 4/22/13 at 10:45 AM, in the presence of Staff #10, it was observed that in the D, E, F basement, the exit stairway was not marked.
2. On 4/23/13 at 2:15 PM, in the presence of Staff #10, it was observed that the exit sign at room #B510 had an incorrect directional chevron.
