Based on record review and interview the facility failed to inform the patient representative of the plan of care and treatment for 1 (P1) of 10 (P1-10) patients sampled. This failed practice has the likelihood that the plan of care and treatment was done without the patient or their representative being able to make a informed decision.
Findings are:
A. Record review of P1's electronic medical chart revealed, no documentation of communication with P1's Medical Power of Attorney (MPOA) regarding P1's plan of care or treatment.
B. Record review of P1's paper medical chart revealed, a signed Medical Power of Attorney.
C. On 3/30/2021 at 3:00pm during interview with S4 (Registered Nurse) confirmed, no documentation in the electronic Medical Chart regarding communication with P1's MPOA about P1's plan of care or treatment.
PATIENT RIGHTS: ADMISSION STATUS NOTIFICATION

Based on record review and interview the facility failed to document that a patient representative has been notified when the patient has been admitted to the facility for 6 (P1,3,4,7,8 and 9) out of 10 (P1-10) patients sampled. This failed practice is likely to cause a delay or a exclusion of a patients' or a patient's representative the ability to make a informed decision.
Findings are:
A. Record review of P1's medical charts revealed, no documentation of a patient representative or family member.
B. On 3/30/2021 at 3:00pm during interview with S4 (Registered Nurse) confirmed, no documentation of a patient representative or family member in P1's chart.
C. Record review of P3's medical charts revealed, no documentation of a patient representative or family member.
D. On 3/30/2021 at 3:15pm during interview with S4 (Registered Nurse) confirmed, no documentation of a patient representative or family member in P3's chart.
E. Record review of P4's medical charts revealed, no documentation of a patient representative or family member.
F. On 3/30/2021 at 3:30pm during interview with S4 (Registered Nurse) confirmed, no documentation of a patient representative or family member in P4's chart.
G. Record review of P7's medical charts revealed, no documentation of a patient representative or family member.
H. On 3/30/2021 at 3:45pm during interview with S4 (Registered Nurse) confirmed, no documentation of a patient representative or family member in P7's chart.
I. Record review of P8's medical charts revealed, no documentation of a patient representative or family member.
J. On 3/30/2021 at 4:00pm during interview with S4 (Registered Nurse) confirmed, no documentation of a patient representative or family member in P8's chart.
K. Record review of P9's medical charts revealed, no documentation of a patient representative or family member.
L. On 3/30/2021 at 4:10pm during interview with S4 (Registered Nurse) confirmed, no documentation of a patient representative or family member in P9's chart.
G. On 3/30/2021 at 4:15pm during interview with S4 (Registered Nurse) confirmed, no documentation of a patient representative or family member were in 6 out of the 10 charts reviewed.
