REASSESSMENT OF A DISCHARGE PLAN

Based on review of medical records, dialysis records, discharge instructions, policy, procedure and staff interviews, it was determined the hospital failed to reassess 2 of 2 acute hemodialysis patient's discharge plans and address the hemodialysis care related to catheter safety, risks, care, infection prevention, dietary and fluid management. (Pt's #10 and 11)

Findings include:

The hospital policy titled Discharge Planning included: "...All staff shall be actively involved in discharge planning...to assure that patients and family are involved and properly prepared for discharge...."

Patient #10:
The patient was admitted on 10/04/12. The patient required hemodialysis which was initiated acutely while he was in the acute care hospital. While at this hospital the patient was transported to an outpatient hemodialysis facility for hemodialysis via a central line hemodialysis catheter. The patient received hemodialysis three times a week on Tuesday, Thursday and Saturday. Documentation in discharge instructions did not include education or review for the patient regarding hemodialysis care related to catheter safety, risks, care, infection prevention, dietary and fluid management. The discharge plan did not include the next hemodialysis appointment for the patient.

Patient #11:
The patient was admitted on 10/19/12. The patient required hemodialysis which was initiated acutely while she was in the acute care hospital. While at this hospital the patient was transported to an outpatient facility for hemodialysis via a central line hemodialysis catheter. The patient received hemodialysis three times a week on Tuesday, Thursday and Saturday. Documentation in the discharge instructions did not include education or review for the patient regarding hemodialysis care related to catheter safety, risks, care, infection prevention, dietary and fluid management.

Employee #4 confirmed on 11/15/12, that the medical records for Pts #10 and 11, did not contain documentation of eduction regarding hemodialysis care and the discharge instructions did not contain information regarding the next hemodialysis appointments.

Employee #1 confirmed on 11/15/12, that this hospital contracted with an outpatient hemodialysis facility to provide dialysis care for the hospital patients and that the patients are transported to the outpatient facility for hemodialysis services.
