Based on observation, document review and staff interview the hospital failed to ensure that outdated drugs and biologicals were not available for patient use in one of one fluid warming cabinet in the Emergency Room.

Findings included:

- Observation on 7/19/10 at 11:20am revealed a fluid warming cabinet in Trauma room #2 that registered 95 degrees Fahrenheit. The warming cabinet contained nine 1000 cubic centimeters (cc) container of Sodium Chloride 9% intravenous (IV) solution. The warmed solutions lacked a date when placed in the warmer or when to be removed from use. The warming cabinet contained one 1000 cubic centimeters (cc) container of Sodium Chloride 9% intravenous (IV) solution with the date of 7/16 written on the outer wrap.

- Document review on 7/20/10 of the information sheet provided by the manufacturer of the warmed fluids directed "...Fluids for injection...The containers, in their overwrap, can remain in the warmer for a period not longer than 14 days at a temperature not to exceed 104 degrees Fahrenheit..."

On 7/19/10 at 11:25am staff C acknowledged nine of the warmed solutions lacked a date when placed in the warmer or when to be removed from use and the solution with the date of 7/16 required removal on 7/16/10.
