Based on record review, observation and interview the hospital failed to follow infection control standards to avoid the transmission of infections.

Finding Include:

Record review of the facility "Isolation Policy" revealed 11/16/2021 that stated:
Transmission-Based Precaution.
D. Staff should educate visitors on applying personal protective equipment (PPE) and performing hand hygiene.
E. If visitors do not comply with transmission-based precautions, staff should escalate through their chain of command (for example, a nurses may escalate to the charge nurse or nurse manager).

On 10/15/2022 at 1130 a female was noted to be sitting in patient (ID#12s) room that was marked as contact isolation, without a gown and gloves. Further review revealed the patient had vancomycin resistant enterococcus (VRE).

Interview with the nurse (ID#55) who was taking care of patient (ID#12), stated the female at the bedside was the patients (ID#12) wife and they talked to her about wearing PPE and she refused. She also confirmed the patient (ID#12) had VRE, and visitors should be wearing PPE.
