Based on observation, record review, and staff interview it was determined the facility failed to ensure that the facilities, supplies and equipment are being maintained to ensure an acceptable level of safety and quality. This deficient practice could affect all patients, staff, and visitors in the areas referenced.

a) Observation on 01/21/25 at approximately 11:30 a.m., revealed a black in color mold or mildew like substance around a sprinkler head and along the wall by the ceiling in the community bathroom across from the nurses station.

b) Observation on 01/21/25 at approximately 11:40 a.m., revealed a black in color mold or mildew like substance along the edge of the shower in the community bathroom around the corner from the nurses station.

c) Interview on 01/21/25 at approximately 12:00 p.m. with the Facility Maintenance Manager verified these findings. The findings were also acknowledged by the CEO and COO at the exit interview on 01/21/25 at approximately 12:30 p.m.
