Based on observation and interview the facility failed to secure 3 of 25 medical gas cylinders from falling whether connected, unconnected, full, or empty. The failed practice had the potential to affect all patients, staff and visitors due to the potential for the cylinders to become a projectile if the cylinder heads were knocked off if the cylinders fell over. Findings follow:
A. While on tour of the facility on 06/11/18 at 12:40 PM, observation showed one unsecured medical gas cylinder next to the crash cart in the Emergency Room next to the nurses' station.
B. During the tour RN #1, verified the medical gas cylinder next to the crash cart was unsecured.
C. While on tour of the facility on 06/11/18 at 1:00 PM, observation showed two unsecured medical gas cylinders in the cross corridor near the 1300 corridor in front the restrooms.
D. During the tour the Chief Nursing Officer verified the two medical gas cylinders in the cross corridor were unsecured.

Based on observation and interview, the facility failed to comply with all provisions in the National Electric Code (NFPA 70, 2011 Edition.) Two of six electrical boxes did not have covers as required or the cover was not properly fastened. The failed practice had the potential to affect all staff working in that area because of the potential shock hazard and potential fire hazard the absence of covers presents. Findings follow:
A. While touring the facility on 06/12/18 at 11:20 AM, observation showed two electrical boxes with no cover in the dry storage closet of the kitchen:
1) one single gang box, next to the refrigeration unit in the dry storage closet, containing a live electrical switch had no cover;
2) one electrical utility box, on the back wall of the dry storage, containing a live connection to a fuse and two small transformers was partially opened and missing two of four screws to secure the cover.
B. During the tour, it was verified by the Director of Maintenance the electrical boxes did not have covers as required.
