PATIENT RIGHTS: NOTICE OF RIGHTS

Based on medical record review and facility document review, it was determined the facility staff failed to ensure a patient received the patient's rights and responsibilities notice for one (1) of seven (7) records reviewed.
The finding included:
On 7/20/2022, Staff Member (SM) #12 assisted the surveyor with navigation of the medical record for Patient #3 for a 1/31/2022 admission. The surveyor requested to review Patient #3's signed document for patient rights and responsibilities. SM #12 was unable to locate the document. SM #12 was able to locate a subsequent signed document for patient rights and responsibilities, but one was not available for the 1/31/2022 admission.
A review of the facility's policy titled, Patient Rights and Responsibilities, reads in part: "...B. Inova Responsibilities...Each Inova facility will: 1. Provide to each patient or his/her representative a summary of the rights set forth in this policy...."
On 7/21/2022, the surveyor discussed the concern with SMs #1 and #5.
