Based on observation and interview, the hospital food service director failed to ensure the daily management of the dietary services which included dating of potentially hazardous foods stored in the refrigerator to prevent the potential of food borne illness.

Findings:

During a tour on 4/15/10 at 10:00 AM of the kitchen, an observation of the meat refrigerator was conducted. There was an approximately 24 inch by 12 inch pan of shrimp and vegetables on the shelf, that was not dated with the date that it was cooled down and placed in the refrigerator.

In a concurrent interview with two dietary staff members, they acknowledged that the pan of shrimp and vegetables should have been dated, and the pan was removed by the staff.

This is a repeat deficiency.

Based on observation and interview, the facility failed to provide a sanitary environment to avoid possible transmission of infection by not ensuring that housekeeping staff followed the correct procedure for sanitizing an operating room (OR) mattress between cases. This failure had the potential to cause a patient to develop a post-operative infection.

Findings:

During an observation in the OR on 4/14/10 at 11:05 AM of housekeeping staff sanitizing an OR mattress that had an armrest, the following was observed:

There was a used foam arm immobilizer laying on the armrest of the mattress. The housekeeping staff lifted up the foam arm immobilizer off of the mattress armrest while cleaning the mattress and armrest with sanitizer. The housekeeping staff then replaced the used arm immobilizer on the wet mattress armrest, recontaminating the mattress.

In an interview on 4/14/10 at 11:45 AM with the OR Director, she stated that the used arm immobilizer should not be placed back on the sanitized mattress armrest, as the armrest would now need to be re-sanitized before use.

This is a repeat deficiency.
