Based on observation and staff interview, the facility failed to ensure that the premises were clean and orderly on two (2) of two (2) days of survey by not ensuring that the physical environment was maintained in a sanitary manner in order to avoid sources and transmission of infections and communicable disease.

Findings include:

1. Observation on 11/14/11 at 10:55 a.m. revealed the following:

a) There were five (5) stained ceiling tile in the food preparation area of the kitchen.

b) Floor moldings, throughout the facility, were not secured to the walls.


2. Observation in the laboratory on 11/14/11 at 2:25 p.m. revealed the following:

a) There were five (5) stained ceiling tiles in the laboratory.

b) An accumulation of dust was observed on four (4) ceiling vents in the laboratory.

c) One (1) bottle of blood stain solution, with an expiration date of 11/22/09, was stored in one corner of the laboratory.
An interview with the Laboratory Director during these observations confirmed the listed areas of concern.


3. Observation on 11/15/11 at 11:35 a.m. revealed the following:

a) The floor area across from Room #3 was missing floor molding. Floor tiles were stained with a yellowish color.

b) Room #15 - There was a wash pan and towel stored on the floor of the bathroom.
A six (6) pack of Albuterol was stored on the floor of the closet.

c) Room #16 - One (1) bar of soap was stored on the handrail outside of the room. The wall behind the bed was plastered with a white colored substance. One (1) staff member was seated on the bed while assisting the resident with the lunch meal.

d) Room #17 - One (1) disposable razor was stored on the window seal of the room.

e) Room #19 - One (1) fracture pan was stored on the floor of the bathroom

Interview and observation with the Director of Nurses on 11/15/11, from 11:35 a.m. until 12:10 p.m., revealed confirmation of the concerns as listed.


Interview on 11/15/11 at 1:15 p.m. with the Director of Nurses revealed that the facility did not have a policy regarding storage of resident care equipment.
