Based on observation, the facility failed to provide readily accessible exit discharge.

Findings include:

On 11/15/11, at 11:00 a.m., it was observed that the exits from the Southeast and Southwest exits do not extend to the public way. Exit discharge consist of a concrete pad approximately four (4) foot (ft) by four (4) ft., and grass surface from the pad to the public way.

NFPA 101 7.7.1 states: Exits shall terminate directly at a public way or at a exterior exit discharge.

NFPA 101 7.1.10.1 states: Means of egress shall be continuously maintained free of all obstructions or impediments to full instant use in the case of fire or other emergency.

Based on record review and interview, the facility failed to conduct fire drills quarterly on each shift.

Findings include:

On 11/15/11, at 12:00 p.m., it was observed that fire drills were not being conducted quarterly on each shift. The Administrator stated that he was aware that they were short on the number of drills conducted and had instated the necessary policy to correct the problem.
