Based on observation during a tour of the facility with the nursing director it was determined that not all drugs were stored as per hospital policy.

Findings were:

The following multidose medications observed in the nurse station medication area were not dated and these had been opened as the vials and bottles were not full per observation Lidocaine 2 vials ; Bumex; Kenalog; immodium; ducosate calcium; metroclopromide and oral pediatric prednisone.
The facility Pharmacy policy entitled "Multi dose vials" requires "all multi dose vials to be dated and initialed when opened : any unused portion will be discarded 28 days after the initial entry date.
Interview with the Nursing director on 3/6/12 confirmed that all the medication containers had been opened and not dated.

Observation during a tour in the kitchen area of the hospital revealed that wall areas and the ceiling had peeling paint, not protecting patient food from contamination.

Findings were:

The walls in the food storage pantry were observed to have crumbling paint with cement exposed. These areas cannot be effectively cleaned. The ceiling in the kitchen had two areas with peeling paint- the first was over and around the pipes that connect to the fire suppression system for the hood and the second area was by the walk in cooler and the ceiling air handling unit. The areas under and around these areas were not protected from flaking paint. Additionally multiple shelving units and cabinets had areas where the paint had been worn and/or peeled off and these areas could not be effectively cleaned.
The above was confirmed in interview with the Administrator and Nursing Director.
