Based on observation and record review, the facility is not providing an exit door in accordance with 7.2.1.4.5 which requires that a door will not require more than 15 pounds of force to release the latch, 30 pounds of force to set the door in motion and 15 pounds of force to open the door to the minimum required width. This deficient practice affects all residents, staff and visitors that my need to evacuate from the 100 wing. This facility has a capacity of 14.
Findings are:
On 10/1/2013, between 1:00 P.M. and 3:30 P.M. the exit door from the 100 wing would not open without excessive force. The latch was able to be operated with minimal force. The door did not open until excessive force was used; door then did open to full swing. According to the facility layout, this was a required exit.
