Based on interview and observation, the facility failed to maintain illuminated exit lights. This deficient practice has the potential to affect approximately 20 occupants in one of seven smoke compartments by delaying egress during an emergency. The facility has the capacity for 12 beds with a census of 2 on the day of survey.
Findings are:
Observations on 8/13/2024 at 3:00 P.M. revealed that the exit light located at the main entrance was not illuminated at the time of the survey.
Interview of Maintenance Personnel on 8/13/2024, at 3:00 P.M. indicated that they were unaware that the exit sign was not illuminated.
Fire Alarm System - Testing and Maintenance

Based on record review and interview, the facility failed to maintain the fire alarm system. This deficient practice has the potential of affecting approximately 20 occupants in one of seven smoke zones, by not notifying occupants of a fire emergency. This facility has a capacity of 12 and census of 2 at the time of the survey.
Findings are:
On 8/13/2024 at 1:15 P.M., record review revealed that the facility failed to provide the required semi-annual testing and maintenance of the fire alarm system. Previous testing and maintenance had been conducted on 1/23/2024.
During interview conducted on 8/13/2024 at 1:15 P.M., Maintenance personnel stated that they were unaware that testing and maintenance for the fire alarm system was out of date.
