Based on observation and interview, the facility does not maintain all doors that are part of a fire barrier to be self-closing.

Findings include:

During facility tour on 04/22/13 and 04/23/13, it was observed that one of the double doors to materials management was held open with a wooden wedge. When closed, these doors protect the corridor providing a fire rating of not less than one-half hour.

This fining was verified with Clinical Operations and Outcomes Coordinator Staff #3 on 04/23/13.

During facility tour on 04/23/13, it was observed that the door to the outside in stairwell "b" did not fully close. The top corner of the door rubbed against the top of the door jamb, preventing it from fully closing.
