Based on observations the facility failed to provide smoke barriers that were constructed with at least a one-hour fire resistance rating for 1of 2 walls. This deficiency affects 2 of 3 smoke compartments and the 9 patients in the facility.
Findings:
During facility tour on November 16, 2010 between 2:00 p.m. and 4:00 p.m., the integrity of the smoke barrier wall was observed to be compromised in the following area.
1. There was a bundle of cables that were not sealed going through the smoke barrier wall above the barrier doors by the nurse station. The bundle consist of 3 white wires and 1red wire.
