Based on clinical record review and interview, it was determined the Facility failed to inform seven of seven (#3, #10, #11, #13, #14, #15 and #19) patients the risks of being transferred to another Facility specific to their diagnosis. The failed practice did not ensure the patients were aware of or accepted the risks of the transfer specific to their diagnosis. The findings follow:
A. Review of Patient #3, #10, #11, #13, #14, #15 and #19's clinical records on 06/06/14 revealed the patients were transferred to other Facilities. Review of Patient #3, #10, #11, #13, #14, #15 and #19's "Consent for Transfer" revealed, "The nature and purpose of the transfer, possible alternative methods of treatment, the risks involved and the possibility of complications have been fully explained to me." There was no evidence the risks of transfer were listed so the Patient/Patient Representative was aware of the risks of the transfer.
B. The findings were confirmed in an interview with the Risk Management/Compliance Officer on 06/06/14 at 1210.
