Based on observation and staff interview, this facility is not providing an all-weather surface from each exit to a public way, affecting all occupants of the facility. Findings are:
1. On 6/7/2011, between 11:15 A.M. and 2:45 P.M., the path of egress from the Northeast exit ended at a concrete pad with the remainder of area to a public way is dirt instead of an all-weather surface. This is marked emergency exit. Maintenance Staff A verified this observation.
2. On 6/7/2011, between 11:15 A.M. and 2:45 P.M., the North Hall is not maintained to be clear and unobstructed. According to the facility layout, this was a required exit. Maintenance Staff A verified this observation.
