Based on interview and document review, the CAH did not securely attach the hydraulic nameplate to the sprinkler riser as required by NFPA 25.
Findings:
Sprinkler system Inspection, Testing, and Maintenance Quarterly reports dated 3/24/16, 7/8/16, 9/27/16, 1/3/17, and annual report dated 6/21/16, were reviewed. All 5 reports documented that the hydraulic nameplate was not attached to the sprinkler riser.
On 1/10/16 at approximately 2:00 PM Staff E confirmed that the hydraulic nameplate was not attached.
NFPA 25 1998 Edition states:
2-2.7* Hydraulic Nameplate. The hydraulic nameplate, if provided, shall be inspected quarterly to verify that it is
attached securely to the sprinkler riser and is legible.
