The inspector observed, while accompanied by the Director of Plant Operations during the hours of the inspection from 1:30 pm to 4:30 pm on 01/24/2017 that there were the following issues.
Based on observation, the facility failed to provide adequate documentation for flame spread ratings for floor, interior wall and ceiling finishes for the general hospital location and the psychiatric hospital location.

The inspector observed, while accompanied by the Director of Plant Operations during the hours of the inspection from 9:00 am to 11:00 am on 01/26/2017 that there were the following issues. The kitchen had both type K and type ABC fire extinguishers. Only type K fire extinguishers are allowed where cooking appliances use combustible media per the following requirement.
''Fire extinguishers installed specifically for the protection of cooking appliances that use combustible cooking media (vegetable or animal oils and fats) without a Class K rating shall be remove from service.''- NFPA 10, 2012: 5.5.5.2.

The inspector observed, while accompanied by the Director of Plant Operations during the hours of the inspection from 1:30 pm to 4:30 pm on 01/24/2017 that the corridor double doors in the basement was missing an astragal.
Smoke barrier doors shall be a minimum of 1 3/4" thick, solid-bonded wood core or equivalent with self-closing or automatic-closing devices in accordance with 21.2.2.4. Latching hardware is not required. Doors are required to swing in the direction of egress travel. Rabbets, bevels, or astragals are at meeting edges, and stops are at the head and sides of door frames. Center mullions are prohibited in smoke barrier door openings 20.3.7.9, 20.3.7.10, 20.3.7.13, 20.3.7.14

They were: Basement: Boiler Room: Control Air Tank: The copper gas lines were not protected from dissimilar metal contact with steel structural members located on the wall.

First floor electrical room: Life safety electrical panel: The fire alarm breaker was not correctly marked per the following requirements. ''The circuit breaker at the electrical panel board for the fire alarm shall have a red marking, shall be accessible only to authorized personnel, and shall be identified as ''FIRE ALARM CIRCUIT CONTROL'' '' - NFPA 72, 2002: 4.4.1.4.2.2.
Second Floor: Electrical Room: There was a ladder blocking the required clear space in front of the electrical panels.

The electrical room failed to have adequate clear space around the panels. There were pallets of used HVAC parts blocking the clear space.
