Facility failed to maintain fire alarm inspections in accordance with the NFPA 72. Findings included the following:

Surveyor observed between 10 and 10: 30 am on 1/14/14 that facilty records did not include annual inspection report for the year 2013 from licensed fire alarm contractor.

Facility failed to maintain electrical panelboards in accordance with NFPA 70. Findings included the following:

Surveyor observed between 9:30 and 10 am on 1/14/14 that electrical panel on wall outside staff corridor was unlocked allowing access by unauthorized individuals.
