Based on record review and interview, the facility failed to provide a safe environment for 1 of 3 patients reviewed (Patient#1) as shown by not providing Patient #1 with a timely required face-to-face examination after being restrained.

Record review of facility policy titled "Policy Manual: Provision of Care-Nursing, Policy No.: POC-N145 ...:, describes Proper use and monitoring of patient restraints, emergency medications and seclusions. Section 5.0 states that a Face-to-Face evaluation within one hour of restraint, seclusion, or emergency medication must be completed by a physician, an authorized nurse practitioner or physician assistant, or trained RN.

Record review of Patient #1's clinical records showed that the patient was restrained and secluded on 2/8/21 at 18:50 because of violent behavior; the patient was slamming and kicking doors, banging his head on windows, and provoking peers. However, records showed the patient did not receive a face-to-face exam until 2/9/21 at 12:52. This far exceeded the one-hour time limit.

In an interview on 3/2/21 at 12:20 PM, RM Staff# B acknowledged that the one hour post restraint face-to-face exam did not occur until the following day, after at least 24 hours post seclusion and restraint.
